This Form should be completed by the LBA Vice-Chair or LBA Secretary. This form cannot be completed by the Pastor.
PLEASE NOTE: Following the updates to The Discipline adopted at General Conference 2022, all pastoral calls are now classified as extended calls. A pastoral review will be conducted every four years by the Local Board of Administration (LBA) in conjunction with the District Superintendent to reaffirm the call (Disc. 782:13).

Under the extended call system, pastors will remain in their positions unless the LBA identifies any issues that require a vote by the Local Church Conference.
